Our society permits a few professions, e.g., medicine, law, and accounting, to self-regulate, mainly by setting standards of ethical and professional conduct and disciplining practitioners who fail to meet them. However, there exist few early indicators of medical professionals, specifically doctors’ unethical behavior, which would be valuable to patients as well as to regulators for effective governance. Increasingly, patients use rating websites as open platforms to provide information on their experiences. These reviews may possess operational values on the management and regulation of professional conduct. Yet few studies have theoretically or empirically established the link between patient reviews of professional services and potential or ongoing ethical breaches in medicine. This research introduces a novel set of text-based measures of physicians’ professional ethics using a large dataset of online patient reviews. Drawing upon the literature on healthcare quality and ethics, we design and validate a novel deep-learning-based approach to quantify ethical-related concerns in patient reviews. We combine a large language model with few-shot learning to accurately extract indicators of professional ethics. Empirical results suggest that our novel measures can help predict drug-related deaths, disciplinary actions, details in malpractice claims, and rent-seeking behavior indicated by the association of pharmaceutical firms’ payments to physicians and their prescription behaviors. Our study provides a new framework for detecting early indications of unethical behavior, allowing healthcare organizations and regulators to promote ethical conduct and prevent medical misconduct.

个人简介:

Professor Dawei (David) Zhang is currently the Class of '61 professor in the Department of Decision and Technology Analytics at the College of Business, Lehigh University. He served as the program director for Business Information Systems at Lehigh and played a pivotal role in co-founding two brand new programs at Lehigh: the undergraduate program in Business Analytics and the Master Program in Business Analytics (MSBA). Currently, as the faculty program director for the MSBA program, he continues to contribute significantly to the growth and success of Lehigh’s business analytics programs.

Professor Zhang's research focuses on the business value of IT, the relationship between IT and labor, technology-enabled business models, and health IS/IT management. His work has been published in top business journals, including Information Systems Research, Journal of Operations Management, Decision Sciences, and Journal of the Association for Information Systems. He has been serving as an associate editor for the Information Technology Management Journal.

In recognition of his research achievements and contributions to the school, Professor Zhang has been honored with the Carl & Ingeborg Beidleman Research Award and the Robert and Christine Staub Faculty Excellence Award at Lehigh University.
